Credit Card Authorization Form for Color Purchases

By completing and submitting this form, I authorize Vanilla Salon to charge my credit card for color product purchases. I understand and agree to the following terms:

Purpose This authorization applies to charges for color product purchases, and retail product purchases made at Vanilla Salon.

Frequency of Charges My card will be charged 2 to 3 times per week, as determined by the stylist's product usage and needs.

Manual Entry I understand that my credit card information will be entered manually for each transaction.

Receipts and Records A receipt for each transaction will be provided via email after each transaction.

Card Security Vanilla Salon will keep the credit card information confidential and securely stored.

Stored Credit Card Information Removal Credit card information will be removed from our database once my employment or contract ends, and all balances are cleared and paid in full.

Charge Disputes Any disputed charges must be reported to Vanilla Salon within 7 days of transaction.

I acknowledge that this authorization will remain valid for the duration of my employment with Vanilla Salon. Furthermore, I authorize Vanilla Salon to charge my card for any remaining balance of color products used after my employment has ended, if applicable.
